Hospital Training

Up to 12 months of General Practice training is hospital based.

During this time GP registrars complete terms that equip them with relevant knowledge and skills for General Practice.

All GP registrars need to complete terms in General Medicine, General Surgery, Accident & Emergency and Paediatrics, before commencing general practice based training. See the IGPE Hospital Manual

GP registrars training with IGPE must do this component of their training in a hospital based in the region.

We will also approve hospital training at:

  • Sydney Chidrens' hospital Randwick
  • The Childrens' Hospital Westmead

There is provision for GP registrars to receive Recognition of Prior Learning for their hospital time. (See the RPL Information Sheet)

We encourage GP registrars to complete their training requirements for Advanced Life Support (ALS) during their hospital training time.

General Practice Based Training

A minimum of 18 months of GP training is undertaken in community General Practice settings. This training is under the supervision of an experienced GP supervisor, who is a mentor and teacher.

IGPE boasts some of the most experienced GP supervisors in NSW with some having ‘clocked up' more than 20 years of having a GP registrar in their practice.

All components of the General Practice based training are compulsory. A minimum of 6 months of GP based training must be in a rural setting and a further 6 months in an outer metropolitan setting. GP registrars have the option of completing these requirements during any component of their general practice based training (ie basic, advanced or subsequent terms). It is important to remain aware of these requirements when planning your GP based training.

The Terms & conditions that you work under during this component of training are negotiated between you and your supervisor. Guidelines are available.

Basic & Advanced GP terms

These are the first General practice based training terms that a GP registrar must complete. They can be undertaken in an urban or rural setting.

They each are 6 months in duration, (longer if you are working part-time). During these terms you are employed to work in the practice by the GP supervisor. This component of training can only be undertaken in practices that are accredited for training through the RACGP.

During these terms you will receive constant support and training from your GP supervisor, in addition you will be paid to attend the half-day educational program delivered through IGPE (see the educational program) or the relevant regional training provider if you are in a rural setting. All GP registrars attend the IGPE workshop program, regardless of the location of their term.

When you are ready to commence your basic or advanced GP term, you will need to complete an application process. (relevant forms are sent to GP registrars twice a year). You will be allocated to an appropriate term by IGPE (or relevant rural RTP if applicable). You are not able to organise your own basic or advanced GP terms, regardless of whether you are planning to undertake this term in an Urban or rural setting.

An introductory seminar is held prior to commencement of your Basic GP term. This will provide you with essential information for this component of your training and attendance is compulsory.

Subsequent General Practice Experience

This component of GP based training can only be undertaken after completion of your basic and advanced terms. It can be undertaken in an urban or rural setting.

It is 12 months in duration, but can be less if you have completed any special skills training (longer if you are working part-time). During these terms you are employed by your GP supervisor who also provides mentorship and education.

You need to plan ahead for your subsequent GP experience training, as you are required to find your own training post. IGPE can assist you in this process. All potential training posts need to be approved before training can commence. Training posts in practices outside the IGPE region will NOT be approved (see Map).

During this training time you are required to attend the IGPE educational program if you are in an urban setting. (different requirements may apply in a rural setting and you will need to contact the appropriate rural RTP).

Special Skills training

This is an optional component of training and can be contribute towards a maximum of 6-months of training time. It can be undertaken in a hospital or community setting, either in Australia or overseas. It is available to all GP registrars and enables them to spend additional time enhancing their knowledge and skills in a special interest area that is relevant to General Practice.

Plan ahead if you want to do a special skills post as approval is required prior to commencing the training post. Further information is available in the Special Skills Information Sheet and the application form .

It is advisable to speak with your medical educator when considering to undertake any special skills posts.

If you undertake any special skills training, the time requirement for Subsequent General Practice is reduced accordingly by a maximum of 6 months.

Mandatory Elective

Mandatory elective is the training time that GP registrars are required to complete if they have;

  1. Completed all the required hospital training (ie Intern and RMO I year) including mandatory terms (see hospital training) and
  2. Are ineligible to receive a reduction in their training time through recognition of prior learning or
  3. Choose not to receive a reduction in their training time through recognition of prior learning, but do not wish to undertake any further hospital training.

Mandatory elective is 12months in duration, and can be undertaken in community based posts, hospital based posts or in general practice.

If you are completing your mandatory elective time in General Practice then the same requirements apply as in subsequent GP time.

Mandatory elective time does not have to be completed as a 12month block but can be distributed throughout your training time. Minimum time to contribute to training is 10 weeks. (ie less than 10 week placements will not count towards training). If you choose to complete any mandatory elective time in General practice it can not occur until you have completed your basic and advanced general practice terms.

Mandatory elective training requires prospective approval from your medical educator.

Leave (and/or elective)

All GP registrars are entitled to a maximum of 2 years leave or elective, this does not include parental leave which is 12 months per child, and is additional to the above leave entitlements.

Leave can be taken at any time during training, except at the commencement of your first training year. (ie you cannot commence your training on leave). However all leave in excess of 6 weeks per year, needs to be discussed with your medical educator and be reasonable with respect to your training placements. For eg it is not reasonable to take 3 months of leave during a 6-month GP term. Leave of this nature will not be approved unless there are exceptional circumstances. Leave outside of annual leave provisions (4 weeks per year) is NOT paid.

A Leave Approval Form needs to be completed and for any leave (other than annual leave). This form needs to be approved by your medical educator.

Occasionally GP registrars work during their leave time. (ie it is not recreational leave), under these circumstances it can be termed elective time, but this time does not contribute to the overall training time of 3 years.

Academic Special skills posts

These positions are available to GP registrars who wish to gain training experience in an academic environment.

There are a limited number of subsidized posts available in conjunction with the General Practice departments of the Universities of NSW and Sydney. Information about applying for these posts and the approval process is available on the GPET website.

To be eligible for these posts GP registrars need to;

  • complete the required application form
  • have the approval of their regional training provider to undertake the post
  • have completed their Basic GP term.
  • Submit their competed application form to GPET, for review by the approval committee.

The posts are a maximum of 6 months full-time and must include a clinical component (minimum of 3 sessions per week).

These posts can be undertaken as part of your 3 years training and count towards special skills, if you have completed your training time but are still interested in undertaking an academic post, provision is available to extend your training time by the required amount, again approval is required through GPET.

Academic Registrar position GP Unit, Fairfield Hospital

This position offers registrars the opportunity to develop skills in research and teaching. Registrars will be supported through the process by senior staff, from formulating a research question and designing a study through to analysis and publication. They will also have the opportunity to assist with undergraduate medical teaching and experience working in an academic department of general practice.

The GP Unit is affiliated with the School of Public Health & Community Medicine, University of New South Wales and the newly established UNSW Research Centre for Primary Health Care and Equity. It has a research focus on chronic disease management, integration of health systems in primary care and improving preventive care in general practice. Staff contribute to teaching in various stages of the undergraduate medical course at UNSW. The unit also provides a clinical general practice service and there is the opportunity for full-time academic registrars to complete their clinical requirements in the position.

Employment opportunity at Kirketon Road Centre

The Kirketon Road Centre (KRC) is a primary health care facility located in Kings Cross, which is involved in the prevention, treatment and care of HIV/AIDs and other transmissible infections among at risk young people, sex workers and injecting drug users. It also operates a low threshold methadone access program (MAP) which targets street-based opioid dependent injecting drug users (IDUs) who have had difficulty accessing or being retained in other methadone treatment programs. This position is responsible for the provision of day-to-day, quality medical care for KRC clients within the framework of primary health care as well assessment and treatment of marginalised IDUs.
